In 2002, the World Health Organization (WHO) introduced the Innovative Care for Chronic Conditions (ICCC) Framework, which emphasizes community and policy components of enhancing chronic disease health care. In 2003, ICIC and a small group of specialists revised the model to add more detailed concepts for each of the six elements.
