Interoperability is an overlapping set of technical and policy challenges, from data access to common data models to information exchange to workflow integration – and these challenges often pose a barrier to healthcare innovation. I got involved with the FHIR community early when I wrote the first open-source FHIR server.

I discussed the current state of the EHR with Michael Nissenbaum of Aprima. The EHR has always been a data collection tool, a mechanism for data aggregation. Aprima belongs to CommonWell, has done some FHIR development, and Michael would like to see Congress condition Medicare reimbursement on real interoperability.
