But these facilities are afterthoughts in our health care system, and were excluded in particular from the huge digitization of health care triggered in the United States by the 2009 HITECH act and 2010 Affordable Care Act. The siloed nature of such data repositories can be attributed to various factors, including using different EHR systems, varying data capture standards, and a historical lack of emphasis on system interoperability. The solution to these challenges lies in the adoption of interoperable data systems.

This seems to me a reasonable way to identify duplicates, although one can question what happens when an EHR automatically generates text. Most hospitals are not set up share their records; interoperability is still limited. Several respondents pointed out that the primary purpose of EHRs is billing, not treatment.
