This care model was refined in 1997 with input from a large panel of experts, and then tested nationally across various healthcare settings in 1998 as part of the program “Improving Chronic Illness Care” (ICIC) funded by The Robert Wood Johnson Foundation. We will discuss the six key elements of the Chronic Care Model in the next sections.
