Discussions at FHIR DevDays raised this question of a basic way to leverage FHIR API capability to enable the Patient beyond the limited Apps their provider has approved. If a healthcare provider offers a FHIR API (e.g. argonaut) -- meeting the Meaningful Use "API" requirement. Would it be reasonable to expect that the Meaningful Use "Download" (from View/Download/Transfer) capability would be offered in FHIR format?

At that time, IHE only had the XDS solution, which is based on Web-Services using SOAP, SAML, and ebRegistry. However this solution is very hard to use if one is using programming tools more common on lightweight systems such as Mobile. So back in 2011 I wrote the first profile in IHE that was targeting ‘ease of use by lightweight application platforms such as Mobile Health Applications”. Thus it targeted use of HTTP RESTful, using JSON encoding.

Tethered PHRs became available when patient portals were launched by hospitals, health systems, and clinics to meet the CMS Meaningful Use requirements. It seems that we're finally entering an era where advances such the FHIR (Fast Healthcare Interoperability Resources) Specification and other interoperability standards will allow patients and providers to link critical health information. FHIR is finally allowing true interoperability to become a reality.

I like it, so will use it here This is such an exciting perspective of what the Wisconsin HIE delivers today, and where they are targeting for future support. The good news is that FHIR has a Document model, and the FHIR Document model has a directly convertible data model A FHIR Document travels an HIE easily CDA will fade, but never disappear.

I am especially excited about the latest standard from HL7 - FHIR. The FHIR standard leverages modern platforms and interaction models. It models the healthcare data-model using XML or JSON; and interaction-model using http REST. I currently hold a co-chair position in HL7 security workgroup, as well as a leadership position in HL7 FHIR Management Governance. Interoperability standards allow for the best re-use of technical implementations.

Predictive risk models can extract data regarding social determinants of health, which can be used in conjunction with structured data.
