FHIR patient extensible data portability

Healthcare Exchange Standards

Discussions at FHIR DevDays raised this question of a basic way to leverage FHIR API capability to enable the Patient beyond the limited Apps their provider has approved. If a healthcare provider offers a FHIR API (e.g. argonaut) -- meeting the Meaningful Use "API" requirement. Would it be reasonable to expect that the Meaningful Use "Download" (from View/Download/Transfer) capability would be offered in FHIR format?

What is MHD beyond XDS-on-FHIR?

Healthcare Exchange Standards

At that time, IHE only had the XDS solution, which is based on Web-Services using SOAP, SAML, and ebRegistry. However this solution is very hard to use if one is using programming tools more common on lightweight systems such as Mobile. So back in 2011 I wrote the first profile in IHE that was targeting ‘ease of use by lightweight application platforms such as Mobile Health Applications”. Thus it targeted use of HTTP RESTful, using JSON encoding.

Tethered PHRs became available when patient portals were launched by hospitals, health systems, and clinics to meet the CMS Meaningful Use requirements. It seems that we're finally entering an era where advances such the FHIR (Fast Healthcare Interoperability Resources) Specification and other interoperability standards will allow patients and providers to link critical health information. FHIR is finally allowing true interoperability to become a reality.

They used the following diagram to show what they viewed as the HIE future. I like it, so will use it here This is such an exciting perspective of what the Wisconsin HIE delivers today, and where they are targeting for future support. The good news is that FHIR has a Document model, and the FHIR Document model has a directly convertible data model A FHIR Document travels an HIE easily CDA will fade, but never disappear.

2019: Healthcare IT gains new ground

Healthcare IT News - Telehealth

Of course, that's been the case every year over the past decade since the first meaningful use checks were mailed out, kickstarting the digital healthcare age as we know it. The National Coordinator talks 21st Century Cures, information blocking, Apple, consumerism, FHIR, open APIs and new business models he sees emerging amid the "overarching theme of human choice and freedom and dignity." How FHIR 4 will drive interoperability progress in healthcare.

Microsoft, Amazon, Google, IBM, Oracle, and Salesforce issue joint statement for healthcare interoperability

Lloyd Price

This requires a variety of technical strategies and ongoing collaboration for the industry to converge and embrace emerging standards for healthcare data interoperability, such as HL7 FHIR and the Argonaut Project. In the context of US healthcare, many health record systems have focused on consistent representation for a key set of data elements defined by the Meaningful Use Common Clinical Data Set.

Looking for career opportunity

Healthcare Exchange Standards

I am especially excited about the latest standard from HL7 - FHIR. The FHIR standard leverages modern platforms and interaction models. It models the healthcare data-model using XML or JSON; and interaction-model using http REST. I currently hold a co-chair position in HL7 security workgroup, as well as a leadership position in HL7 FHIR Management Governance. Interoperability standards allow for the best re-use of technical implementations.

This selection focuses on collecting and understanding data, using it to create information that can be used to improve health and healthcare. Now, as Michael said, the EHR is taking in data, analyzing it, and enabling its use back in the practice as an adjunct to the physician decisionmaking process, using data to diagnose and treat. Predictive risk models can extract data regarding social determinants of health, which can be used in conjunction with structured data.

Simplifying Security Audit Standards

NIST SP800-92 – shows how to do audit log management and reporting – consistent with our model HL7 PASS – Defined an Audit Service with responsibilities and a query interface for reporting use HL7 FHIR - Defined a RESTful binding for the Security Audit Event, and thus provide a Query/Retrieve also ISO 27789 – is defining the subset of audit events that an EHR would need There is very little about IHE-ATNA specification. I am however happy to use IHE-ATNA as the marketing tag.

Data from smartphone applications (colloquially known as “apps”) and an ever-growing range of wearable and environmental sensors can be processed with the use of machine learning and other AI techniques to support medical decision making. I do not discuss sensors and apps intended solely for use by health care professionals in health care settings. 40 In one survey, more than half of users of activity trackers stopped using their device, and a third did so in the first 6 months.
