Question : I have a simple question, does R4 have the capability of storing video type data back into the EHR? And lastly, is there a timeline of when specific EHR vendors will implement this capability into their EHRs? Answer : There’s several parts to this answer: FHIR Support for Video. Yes, the FHIR specification supports the Media Resource. Timeline for EHR support. I haven’t heard anything about existing EHRs supporting video.

FHIR Foundation member Wes Rishel ( @wrishel ), who’s a FHIR user, and also was a Gartner Analyst before he retired, graciously made this contribution that I could post here. In the last few years I created and positioned a spot for FHIR. FHIR could be at the top of the slope while V2 messages still flow.

IHE has created a FHIR based Patient identity management system for health information exchanges. It is possible that a formal Merge will be supported, but in FHIR a Merge comes with some downsides. When FHIR is a front-end to a non-FHIR system (e.g. an EHR), the result will likely look like a Merge, in that only the surviving identity will ever be seen from again. Especially if those FHIR Resources have digital signatures on them.

I’m very honoured to make a guest today from Mike Morris, who I met at the HL7 FHIR Applications Round Table in Washington DC a couple of weeks ago. Mike is a cancer patient who is using FHIR improve his own treatment. I saw all the technical issues during my cancer fight – data siloed in different EHRs, CDs/DVDs of my scans somewhere else and an industry that STILL likes to use fax machines and emails to get things done. Data doesn’t stop with EHRs.

I think the most useful value-add that an HIE can add is an API that is based on FHIR. This is true of an XDS based HIE, Regional Exchange (XCA), Vendor based EHR, nationwide Exchange, and Direct HISP. A Document Sharing environment can share FHIR documents. There is no special magic in the IHE PDQm profile, it is just a FHIR Patient. It can also happen when the API is being served by an EHR, PHR, or other data source. Also see my FHIR Topic

Apple recently announced that its mobile Health app will link to electronic health record (EHR) systems. While companies like Microsoft launched PHR platforms such as HealthVault a number of years ago, most of these PHRs were not linked to EHRs. It seems that we're finally entering an era where advances such the FHIR (Fast Healthcare Interoperability Resources) Specification and other interoperability standards will allow patients and providers to link critical health information.

FHIR has similar problems to HL7 in that the content communicated using FHIR is still highly variable and specific to each health system.
