Intermountain Healthcare and virtual specialty care company Story Health announced a strategic partnership to allow the health system's patients with heart failure to access care outside the clinic.

Calif.-based Story Health offers telehealth, health coaches, AI and connected devices to treat patients with heart failure at home. It also lets doctors access patient data and receive personalized recommendations based on a physician's historical prescribing patterns.

As part of the partnership, Intermountain clinicians and the health tech company's coaches will collaborate to provide personalized treatment to patients and monitor medication access and adherence between visits.

The coaches will act as the go-between for patients and their providers, monitoring patient progress and answering questions via text, email or call.

"Patients have long journeys with their health, and a significant part of that journey takes place after they leave the clinician's office," Tom Stanis, CEO and cofounder of Story Health, said in a statement. "Our solution combines the latest AI-integrated electronic health record technology with real health coaches to directly identify and address the most disruptive barriers to a disconnected patient journey. Story Health is excited to be partnering with Intermountain to transform continuous care for patients."

THE LARGER TREND

Story Health received $22.6 million in Series A funding last year, which the company said it would use to expand its reach into the cardiology space. The investment came after the company launched with a $4 million seed funding round in late 2020.

In October, Intermountain Healthcare announced a partnership with autonomous drone delivery company Zipline to deliver prescriptions and medical products to patients in the Salt Lake Valley area of Utah.
