Chronic Care Management – #HITsm Chat Topic

We’re excited to share the topic and questions for this week’s #HITsm chat happening Friday, 8/16 at Noon ET (9 AM PT). This week’s chat will be hosted by Brian Eastwood (@Brian_Eastwood) on the topic “Chronic Care Management”.

We know the numbers: Americans with chronic health conditions account for the majority of the nation’s $3.5 trillion in health costs. While some of these costs are unavoidable, many can be avoided through a combination of earlier diagnosis, improved treatment, better support outside the clinical setting, or prevention (when possible).

Technology stakeholders across health care approach the challenges of improving chronic care management from several angles. These range from individualized tech-enabled programs to large-scale research efforts focused on providing more effective care to millions of Americans with the same condition. Some approaches are in their early stages, while others are beginning to mature. What holds the most potential – and where do challenges remain?
